Healthy Families Tennessee
Healthy Families Tennessee is a proud recipient of one of five Community Home Visiting Grants through the TN Department of Health, to serve families in Davidson County. This program of PCAT was selected for this funding due to the strong adherence and committment to evidence based, effective services to parents and children.
Healthy Families Tennessee offers in home parent coaching, support and information to expectant parents and families with at-risk newborns. Though parents can self refer to Healthy Families Tennessee, the program primarily receives referrals from other community agencies, such as area hospitals, local health departments and mental health centers. Family support counselors offer emotional support, discuss infant care and development, model parenting skills, help maintain sobriety when needed, and link families with community resources.
Parents are eligible fore the program if they are in the last trimester of pregnancy or if the child is less than three months old. Healthy Families Tennessee counselors remain with the family for up to 5 years, working to build strengths and support networks.
